The appearance of diverticulosis can usually be connected to eating habits and chronic constipation. This disease is caused by small, pouch-like intestinal formations, the so called diverticula. These pouches can be found on the inside wall of the digestive system, mostly in the colon and its weakened areas. Serious health problems arise only when the diverticula become infected. In such cases colon inflammation, that is diverticulitis develops.

The main reason behind diverticular diseases is a diet without sufficient amount of fibre. This disease has been known ever since people started to change their eating habits and the technologically processed foods and products lacking fibre appeared. Over and above this, the intake of fruit, vegetables and legumes has decreased. For this reason, colon inflammation is found mostly in the developed areas of Europe, America, in Australia, while it only occurs in a case-by-case basis among the inhabitants of Asia and Africa.

What causes colon inflammation?

Who is especially at risk? The answer is obvious… Do you prefer consuming white to whole wheat bread? Do you consume less than a pound of raw or steamed vegetables and some fruit every day? Have you been suffering from chronic constipation for a long time? Are you over 40 years of age? If the answer to each and every of the previous questions is yes, you are among the ones selected for diverticula formation.

The most sensitive spot of the digestive tract is the large intenstine. The small pouches in the mucous lining are at the centre of the disease; they press the muscles of the large intestine apart, and once appeared, they are there to remain. If there are many of them appearing, then it is the disease referred to as diverticulosis. The highest number of the outpouchings is developed in the area with the strongest pressure during intestinal evacuation. This mostly means the area of the “s” curve and around the left downward part of the large intestine, as while firm stool is depleted, the pressure increases in the intestine and, in its weakened areas, the mucous lining wides out.

Diverticulosis is practically a type of hernia, and we know that it surfaces as small pouches that form on the wall of the large intestine. They can be compared to the tiny balloons forming at worn-out spots on a bicycle tyre. The size of diverticula are from 3 mm to even more than 3 cm. The percentage of incidence increases with age and they rarely occur in people of less than 40 years of age, but after reaching 80 years nearly everybody develops them. Why? Because a diet lacking in fibre, chronic constipation, obesity, the weakening connective tissue, impaired immune system, and chemotherapy all contribute to their appearance.

Diverticulosis is strictly related to the eating habits and lifestyle. Most important in this regard is that our food does not include a sufficient amount of fibre, resulting in chronic constipation. If there is an optimum amount of roughage, the stool becomes softer, the inner pressure in the intestines eases, and the bowel movement becomes more relaxed. Thus an increased intake of fibre may detain, or at least decrease the incidence of colon inflammation and, as a result, one can avoid the adverse effects of diverticulitis.

The symptoms of diverticulosis

Usually there are no special abnormalities. If difficulties do occur, they are in the form of a feeling of fullness, a slight pain in the stomach or lower abdomen, bloating and problems with defecation. However, such complication can be quite severe. This might occur when a piece of undigested food is caught in one of the diverticula, then they get infected, thus leading to diverticulitis, i.e. colon inflammation, which can spread into a bigger area as well. The condition is characterized usually by increased body temperature, accompanied by pain in the left part of the abdominal cavity. Often a sudden bleeding from the rectum indicates the infection.

Potential problems if not taking timely action

Diverticulosis may be the reason of bleeding into the stool, because colon inflammation may destroy blood vessels and lead to heavy bleeding, and inflamed pouches may even get perforated. In such a case the infected part of the intestine enters the abdominal cavity which causes there a life threatening inflammation of the peritoneum. In such cases the stomach is extremely painful, taut and hard, and choking may also occur. Gasses cannot escape and the body temperature may also rise.

The inflammation may also take a slower course; in such a case it may gradually spread to the neighbouring organs. This is a further complication whereby fistulae come into existence, which are abnormal links between the intestine and other organs. Intestinal bacteria may spread through them to the other organs. In women, a connection between the colon and the vagina is often formed. This is indicated by a festering vaginal discharge, or bacteria in the urine. Pain and a burning sensation accompany urinating, and these recur even after it has been treated with antibiotics.

How may one proceed

If you experience bleeding into the stool, stomach ache, and other symptoms of diverticulitis, do not hesitate at all to contact a doctor! Applying an ice pack to the lower left abdominal area can rarely aleviate pain. Never apply a warm compress as this accelerates the process of the infection and can lead to a perforation of the pouches.

The milder symptoms can be treated by using antibiotics and a liquid diet; these do not burden the large intestine. Difficulties will generally ease off in a matter of a couple of days. It is necessary to change for a gentle diet and eat mashed food only, i.e. in such a case do not eat raw fruit, fruit including pips, or vegetables, whole-grain bread or cakes, hot spices, maize/corn; and further, do not drink beverages containing alcohol or caffeine.

Contact emergency in the case of an acute colon inflammation – diverticulitis, or if the intestinal wall has been ruptured (perforation of the intestine) leading to the inflammation of the peritoneum (peritonitis). In such a case the closest emergency services must be contacted immediately, as most often, a hospital intervention, or even an operation becomes necessary. Do not drink or eat anything until the examination.

Is it possible to prevent further complications?

The best prevention is eating food of a high fibre content. As a result a higher amount of stool is formed and thus the large intestine does not need to perform extreme contractions during defecation. As a result the formation of the pouches or pushing the stool into the existing pouches can be prevented; this latter could not only gradually enlarge them but later on it might also bring about colon inflammation.

Thus, fibre plays a significant role in the prevention of complications that can be inflammations or even a subsequent threat of a perforated intestine. If the doctor confirms a diagnosis of this illness, he/she will most probably recommend a change in eating habits first to avoid an operation, which is int he majority of the cases inevitable if such problem arises. It is recommended to avoid eating processed food which can result in constipation, and is the primary reason behind diverticulitis, and consume food with a high fibre content.

Change your eating habits

The best method for maintaining good health and preventing health issues is to eat foods with a high fibre content. Legumes, whole wheat bread and products made from unpolished grains are an perfect source. Regularly consume vegetables and fruit in any form, but the fresh, original version is th ebest, because their juice does not contain any fibre. Dried fruit is a great source: prunes, raisins, dates, apricots.

Notice: fibre is important for the body, but gradually increase the amount of it, and slowly change your eating habits. Plan the change over a period of one to two months in advance, so that the digestive system is able to adapt to the change. If you previously consumed foods low in fibre, now you will usually experience bloating and excessive gas during the first weeks due to the increased amount of fibre.

Psyllium – a source of fibre

If in our everyday life we do not consume enough fibre, supplement becomes necessary. The natural source for that is psyllium, which is produced from the husks of the seeds of the Indian plantago. When mixed with a liquid it swells up, becomes gelatinous and increases its bulk to almost 40 times of its original size. Psyllium is a source of soluble fibre, just like oats and barley. The difference is that the fibre in psyllium has more special contents than, for example, oats. Using psyllium often normalizes the functioning of the digestive tract – especially of the small and large intestine.

Fibre absorbs a lot of water, thus it increases the volume of the intestine and stimulates intestinal peristalsis. It can be prepared as follows: put one spoonful into 2 dl of liquid (this can be water, fruit juice, milk or even yoghurt or buttermilk), mix and drink without delay. Afterwards drink 2-3 dl of water. It is very important to stick to a drinking regime of 2-3 litres of water daily!

Which foods are to be avoided with diverticulosis?

Consume less processed food products. Generally speaking they are not healthy at all, but to sufferers of diverticulosis they are especially harmful. Such as: fatty foods, meat, bread and bakery products made from white flour, sweets. Also avoid fruit or vegetables with a lot of pips (strawberries, raspberries, blackberries, tomatoes, figs, etc) either, as pips can get caught in the pouches and result in inflammation.

After a serious case of diverticulitis these kinds of food are not recommemded at all.

Avoid caffeine and carbonated drinks as they irritate the intestine, and consume alcohol only in low quantities. One alcoholic drink a day (a glass of low alcohol drink) helps the colon to relax which can have a positive effect on the outpouchings.
